I hereby authorize Eau Claire Diesel to do the repair work hereinafter set forth, along with using the necessary

materials. I agree that Eau Claire Diesel is not responsible for loss or damage to my vehicle, or articles left in

vehicle, in the event of fire, theft or any other cause beyond their control.

I understand that Eau Claire Diesel is not responsible for delays caused by unavailability of or delays in

receiving parts needed to complete repairs.

I hereby grant permission for Eau Claire Diesel employees to operate my vehicle as needed for testing and

diagnostics inspection.

I acknowledge an express Mechanic’s Lien of the vehicle to secure the financial total of repairs completed.

I understand, that upon completion of repairs, I am required to pay for the work done within two weeks after

repair is complete by invoice date. After this time, there will be $25 per day storage fee, unless otherwise

agreed upon with service manager.

Eau Claire Diesel Warranty Notice:

Eau Claire Diesel Warranty is one-year on technician workmanship, and two-year on all Alliant Power parts.

All parts from other venders subject to vendor specific warranties.

Eau Claire Diesel will not perform work of any kind on vehicles and equipment that are not emissions

compliant. Should the vehicle become non-compliant after work is done by Eau Claire Diesel, all

workmanship and warranty will be void.
